Air Asia Flight QZ8501 crashed in December 2014 killing 162 people was the result both computer and human errors.

Indonesia's National Transport Safety agency published a report following a 12-month investigation. The computer error was apparently with the Rudder Travel Limited system, which controls the rudder's movement.
